The standard is 13 years and older

There is no standard age. The CDT Manual states if there is no primary teeth, it is an adult dentition therefore is coded as an adult prophylaxis. In the case of missing adult teeth where the primary tooth remains in place, it is then considered an adult prophylaxis.
